According to the CDC’s Health Alert Network, several states have reported an increase in acetyl fentanyl-related fatalities since 2012.

Toxicity

There are currently no clinical studies examining the pharmacologic effects of acetyl fentanyl in humans.

Its toxicity is comparable to that of other opioids, characterized by lethargy progressing to coma and respiratory depression manifesting as apnea, bradycardia, and hypotension.  Typically, death occurs as a result of respiratory arrest. The CDC recommends testing patients who exhibit symptoms consistent with opioid overdose with an enzyme-linked immunosorbent assay (ELISA). Naloxone comes in a variety of dosage forms, including intravenous (IV), injectable (IM/SC), and nasal spray. 0.1 to 0.2 mg IV is a typical starting dose. The onset is rapid, occurring within minutes if not seconds. If no response is observed, repeat or increase the dose every 2 to 3 minutes until the respiratory status improves. Because acetyl fentanyl is so potent, doses of 2 to 5 mg or more may be required.

Certain formulations of naloxone are now available for use in non-clinical settings to treat overdoses. Additional treatment is dependent on supportive care, which includes cardiopulmonary resuscitation and cardiopulmonary support. While awaiting a response, the use of naloxone does not preclude continued respiratory assistance. Intubation and mechanical ventilation may be necessary in patients who do not respond to naloxone, especially if cointoxicants are suspected or the picture is unclear. Twenty-one Regulatory Concerns

By testing a drug batch with a single fentanyl test strip, the user obtains valuable information that can be used to best assist that user in their unique situation: The user has the option of using a different batch, using less of the current batch, using in the presence of others, or having naloxone (an opioid reversal agent) on hand. Additionally, the results can provide insight into a new dealer’s trustworthiness, which can inform future risk avoidance decisions or foster caution among the user’s peers. In this instance, increased knowledge equates to increased power. While the strips do not provide precise information about the amount of fentanyl present, testing can provide a user with enough time, space, and information to consider their options. This in and of itself can foster a greater sense of control over one’s health, resulting in safer behaviors and practices. Nonetheless, the widespread stigma associated with PWUD has created barriers to FTS and other harm-reduction tools. Negative perceptions of PWUD are fueled by a lack of compassion for the reasons individuals use drugs in the first place, which contributes to the belief that they lack a moral compass, are untrustworthy, or are unconcerned about their own well-being. These sentiments exist not only in our immediate communities, but also at some of our government’s highest levels. Before you begin using this medication and each time you receive a refill, read the Medication Guide and, if available, the Instructions for Use provided by your pharmacist. Learn how to use, store, and discard the patches properly. Consult your physician or pharmacist if you have any questions.

Use this medication on a regular schedule, not as needed for severe (breakthrough) pain, as directed by your doctor.

Before you begin using this medication, consult your doctor or pharmacist to determine whether you should discontinue or modify your use of any other opioid medication (s). It may take up to 24 hours for fentanyl patches to provide pain relief. Additional analgesics (such as acetaminophen or ibuprofen) may be prescribed. Consult your doctor or pharmacist for information on safely combining fentanyl with other medications.

As directed by your doctor, apply this medication to the skin. Apply sparingly to burns, cuts, irritated skin, or skin exposed to radiation (x-ray treatment). Choose a dry, non-hairy area of your body that is flat, such as your chest, sides, back, or upper arms. Apply the patch to the upper back of young children or people who are unable to think clearly (such as those suffering from dementia) to reduce the likelihood that it will be removed or placed in the mouth. If hair is present on the skin, use scissors to clip the hair as close to the skin as possible. Shave hair sparingly as this may cause skin irritation. If necessary, clean the area with water. On the application site, avoid using soap, oils, lotions, or alcohol. Before applying the patch, thoroughly dry the skin.

Typically, the patch is changed every 72 hours. Apply to a different area each time to avoid irritation. Ensure that you remove the previous patch before applying the new one. Because the used patch still contains enough fentanyl to cause serious harm, if not death, to a child or pet, fold it in half with the sticky sides facing each other and discard properly. Allow no children to see you apply a patch, and avoid placing the patch in an area where children can see it. Children have discovered patches that had fallen off or patches that had been removed from sleeping adults and placed them in their mouths or on their bodies, resulting in fatalities. If the patch appears to be broken, cut, or damaged, do not use it. Remove the sealed pouch, peel away the protective liner, and apply to the skin immediately. Stopping this medication abruptly may cause withdrawal symptoms, especially if you have been taking it for a long period of time or in high doses. To avoid withdrawal, your doctor may gradually decrease your dose. Notify your doctor or pharmacist immediately if you experience withdrawal symptoms such as restlessness, mental/mood changes (including anxiety, difficulty sleeping, suicidal thoughts), watering eyes, runny nose, nausea, diarrhea, sweating, muscle aches, or abrupt behavioral changes.

When this medication is used for an extended period of time, it may lose its effectiveness. Consult your physician if this medication ceases to work properly.

While this medication benefits a large number of people, it can occasionally result in addiction. This risk may be increased if you have a substance use disorder (such as excessive drug/alcohol use or addiction). To minimize the risk of addiction, take this medication exactly as prescribed. It is possible to experience nausea, vomiting, constipation, lightheadedness, dizziness, drowsiness, or headache. Mild irritation, itching, or redness may also occur at the application site. This medication may cause an increase in serotonin levels and, in rare cases, a life-threatening condition called serotonin syndrome/toxicity. Most potent opioid

The opiates that will be examined will be ranked according to their potency. The more potent the opioid, the greater the risk of overdose and the greater the likelihood that a person will develop a physical addiction to the drug.

 

Carfentanil is a highly dangerous opioid that is 10,000 times stronger than morphine and 100 times stronger than fentanyl. Carfentanil is a tranquilizer used to sedate elephants and other large animals and is not intended for human use. Drug dealers may incorporate carfentanil into their drug supplies in order to provide a more potent, addictive high to their customers.

2. Fentanyl – Fentanyl is 50–100 times more potent than morphine and has been a major factor in the recent increase in overdose deaths. It is marketed under the trade names Sublimaze, Duragesic, and Actiq. Additionally, it is referred to as China Death, TNT, Murder 8, Jackpot, Goodfella, Dance Fever, Apache, and China Girl on the street.

3. Buprenorphine (Butrans) – Buprenorphine is a medication approved by the Food and Drug Administration to treat opioid use disorder. It is approximately 25–100 times more potent than morphine, but lacks the euphoric high associated with other opioids. Buprenorphine is used in drug detox to alleviate and reduce the symptoms of opioid withdrawal.

4. Oxymorphone – Oxymorphone is a prescription medication that is marketed under the brand names Opana and Numorphone. It is a long-acting opioid with numerous street names such as O Bomb, Stop Signs, Biscuits, and Blues. Oxymorphone is used to treat patients with moderate to severe pain who are unable to reduce or control their pain with other medications. Oxymorphone is three times as strong as morphine when taken as a tablet and ten times as strong when given as an intravenous injection.

5. Hydromorphone – According to the DEA, hydromorphone is used to treat moderate to severe pain and is approximately two to eight times more potent than morphine. It comes in the following forms: injection, tablet, liquid, and rectal suppository.

6. Heroin – Heroin is a highly addictive illicit drug classified as a Schedule I substance. Heroin is typically two to five times as powerful as morphine and is frequently combined with other highly addictive substances such as fentanyl or carfentanil. In 2018, heroin contributed to 14,996 overdose deaths in the United States.

Methadone (Dolophine, Methadose) is another FDA-approved medication for the treatment of opioid addiction. It is approximately three times stronger than morphine. 8. OxyContin – OxyContin is the brand name for the well-known extended-release opioid Oxycodone. Oxycodone is a prescription medication used to treat moderate to severe pain. It is approximately 50% stronger than morphine. According to the CDC, oxycodone is one of the most frequently used drugs in prescription opioid overdose deaths. In 2018, prescription opioids such as oxycodone were responsible for 14,975 overdose deaths in the United States.

9. Percocet – This medication contains the active ingredients oxycodone and acetaminophen. It is available as a tablet, a liquid oral solution, or a capsule. Percs and Hillbilly Heroin are some of the street names for the drug, which also contains oxycodone.

10. Hydrocodone – Hydrocodone is the next strongest opioid on the list, coming in at number ten. It is marketed under a variety of brand names, including Norco, Vicodin, and Zohydro. It is typically marketed as a way to manage pain associated with surgery, chronic pain, or an injury. It is available in both syrup and tablet form. Hydrocodone is roughly equivalent to morphine in strength and is frequently combined with other analgesics such as ibuprofen and acetaminophen.

11. Morphine – Known as the “soldier’s disease” due to the addictions it caused during wartime, morphine is the next opiates on our list. Among the brand names are MS Contin and Duramorph. 12. Tramadol (Ultram) – Tramadol is an opioid analgesic used to treat moderate to moderately severe pain. It is approximately one-tenth the potency of morphine. This is the only opioid on the Controlled Substances Act that is classified as a Schedule IV substance.

Demerol (Meperidine) – 13. Demerol is the most frequently prescribed brand-name opioid for moderate to severe pain and is approximately 7 to 10 times less potent than morphine. Due to its potency, it may also be used as an anesthetic. The medication is available in a number of dosage forms, including injectable solution, tablet, and liquid oral solution. Pain Killer and Demmies are two common street names.

Though meperidine is less potent than many other opioids, it is classified with oxycodone and fentanyl as a Schedule II narcotic.

Codeine is a relatively short-acting opioid. This medication is frequently prescribed in conjunction with aspirin and acetaminophen. The drug is available in tablet, capsule, and liquid form. When sold illegally, Codeine is marketed under the names Purple Drank, Lean, Cody, and Sizzurp.
